o List the specific agency/program and details regarding goals and anticipated outcomes

o Please provide agencies name and contact information

• Need to include one resource per social problem; the third is your choice.

• Include Reputable References b. Intervention Strategies:

o Identify three (3) intervention strategies to address the social problems you identified (one each; the third is your choice).

▪ Provide details related to the aims/goals of the intervention and the processes/activities used to accomplish such goals.

o Please provide agencies/service provider’s name and contact information o Make sure to demonstrate a clear link between the strategies and the social problem

being addressed. ▪ This section may seem similar to the resource; however, it is asking for specific
